INSTRUCTIONS:
To submit your forms electronically, which is the preferred method, download the PDF form(s) below, fill in the appropriate fields and save the document. Then, to submit your saved PDF form(s), go to the CONTACT US page, fill out the contact form, upload your completed files, and submit.
To print forms shown below, click on the link and then click the print icon in your browser. Alternatively, you can download the form and fill it out, then print completed forms and mail to: CACFP 107 S Division, Spokane, WA 99202.

Holiday Forms:
Holiday notes are required when you have children in care on days listed below. The note should list the child(ren), the date, the times that the child(ren) was in care, daycare provider’s name and parent signature. Don’t forget to send them at claim submission time!
